Application & Eligibility Confirmation

The individual or a referring agency contacts Southbridge to express interest, and a brief screening confirms eligibility based on age and homelessness status, with priority given to senior veterans.

Documentation & Verification

Instead of income or credit checks, Southbridge collects basic identification and residency verification, offering assistance to applicants who need help obtaining necessary paperwork.

Move-In & Ongoing Support

The resident moves in with assistance as needed, and Southbridge offers continued check-ins and community support to promote long-term stability, well-being and independence.

Needs Assessment & Interview

A Southbridge team member meets with the applicant to assess their housing needs, discuss accommodations, and connect them with available support services for a smooth transition.
